Kids Skating Program – Kendall
Service Description
Weekly skating lessons for kids ages 5–10. The program is structured by levels, with each level lasting approximately 4 months. Lessons focus on balance, safety, coordination, and confidence in a fun and supportive group environment.

**Acknowledgment of Risk and Release of Liability** BySCHEDULING THIS SESSION this Liability Waiver and Release Form, I, the undersigned participant (or parent/legal guardian, if the participant is under 18), acknowledge and agree to the following: 1. **Assumption of Risk:** I understand and acknowledge that skating activities, including lessons and practice, involve inherent risks, including but not limited to falls, collisions, and injuries. I voluntarily assume all risks associated with participation in any activity at \[Skating Academy Name], including personal injury, property damage, or death. 2. **Release of Liability:** I hereby release, waive, discharge, and covenant not to sue \[Skating Academy Name], its owners, instructors, employees, agents, and representatives (collectively referred to as “Releasees”) from any and all liability, claims, demands, or causes of action arising out of or related to participation in skating activities or use of the facilities, whether caused by negligence or otherwise. 3. **Indemnification:** I agree to indemnify and hold harmless the Releasees from any claims, actions, suits, damages, and expenses, including attorney’s fees, arising from participation in activities at \[Skating Academy Name]. 4. **Medical Consent:** I authorize \[Skating Academy Name] and its staff to provide or obtain emergency medical treatment for the participant in the event of an injury or illness. I accept full financial responsibility for any medical expenses incurred. 5. **Photography and Media Release:** I grant permission for [Skating Academy Name] to use photographs, videos, or other media of the participant for promotional purposes without compensation. Acknowledgment of Rules and Policies: I agree to comply with all rules, policies, and instructions provided by [Skating Academy Name]. Failure to do so may result in termination of participation without a refund. Parent/Guardian Consent (if applicable): I certify that I am the parent or legal guardian of the minor participant named above and have the authority to sign this waiver on their behalf. I acknowledge that I have read and understood this document and agree to its terms.
